Enrolling in Benefits

When you enroll or update your benefits in 2021, you will need to review all of the benefits and approve your selections. If you are not making any changes, no action is required of you. Please review the following instructional video to update your benefits.

  • I don't have an account. How do I register for myADP.com?
    Select “NEW USER? CREATE ACCOUNT” Enter your registration pass code: caravanfm-4321 Click the NEXT button and follow on-screen instructions. If having trouble logging in, contact the HR Generalist at 989-484-9784
  • What should I do if I cannot remember my user ID for myADP.com?
    Click “FORGOT YOUR USER ID?” at the bottom, and follow the instructions. If having trouble logging in, contact the HR Generalist at 989-484-9784
  • What should I do if I cannot remember my password?
    After entering your USER ID, click “FORGOT YOUR PASSWORD?” Then, follow the prompts. If you have trouble logging in, contact the HR Generalist at 989-484-9784.
  • Who can I add to my insurance coverage?
    Spouse, child, stepchild, adopted child, and child for whom you have guardianship
  • Who cannot be added to my insurance?
    Non-married partner Girlfriend / boyfriend Child of non-married partner (if you are not listed on the birth certificate or paying child support on) Parent Siblings **Anyone other than spouse, child, stepchild, adopted child, child for whom you have guardianship
  • What if I do not know my dependents Social Security Number?
    This is a required field, so it is best to save your benefit changes without confirming. Gather the required SSN and enter it into the system. Once the SSN is entered, complete the enrollment, and confirm the changes. Benefit coverage is reported to the IRS on the 1095C form. It is important that the SSN be correct. DO NOT ENTER AN INCORRECT SSN and DO NOT ENTER THE SAME SSN FOR ALL OF YOUR DEPENDENTS.
  • Why does it ask if my dependent is a full-time student?
    Supplemental Child life coverage ends at age 19, unless the child is a full-time student and financially dependent upon you. If this box is not checked, and the child is 19 or older, you will not be able to enroll the child in Supplemental Child Life Coverage. (Please note that the Insurance company will require proof of financial dependency before paying any claims.)
  • The benefit confirmation page says my dependent's coverage is pending documentation, what does that mean?"
    Caravan uses a Dependent Verification System. You will be prompted to upload proof that the dependent is your dependent.
  • What documents are required to verify my spouse?
    A copy of your marriage license or a copy of your Federal 1040 that shows your spouse as a dependent
  • What documents are required to verify my child/stepchild?
    A copy of the child’s birth certificate showing you or your spouse as the parent, legal adoption documentation, child support order (must come direct from the court) or legal guardianship documents
  • How do I upload documents for dependent verification?
    You will have to upload documents through one of the following means: 1. After confirming benefits, the option to Upload now will pop up through the system. 2. You will receive additional instructions by mail. 3. Fax documents Benefits to 989-484-9025 4. Scan and email documents to benefits@caravanfm.com 5. Call the benefits office: 989-484-9789
  • What do I do if am unable to confirm the benefits?
    Be sure that... 1. You have primary beneficiary(ies) that equal 100%. 2. Each benefit tile has been reviewed and confirmed. Go back and select the first tile (Medical) and confirm Continue to review/confirm each item.
  • I already confirmed my benefits. Can I make a change?
    Yes, if the change is made before 3/7/2021. You will need to confirm each benefit tile again and get another confirmation number.
  • What if I do not have any changes to make?
    Your benefits will remain the same. Caravan still recommends reviewing your current benefits and reviewing your beneficiary on file.
  • Why is the Annual Enrollment Event closed and not allowing me to make changes to my benefits?
    Note: The enrollment period opens 2/15/21 and closes 3/7/2021. If you are unable to update your benefits within the Annual enrollment dates of 2/15/21-3/7/21, please call the benefits office at 989-484-9789.
  • Why can I only make changes once a year?
    The rule is due to regulatory compliance for group health plans. Therefore, you can only make changes once a year, unless you have a Qualifying Life event such as marriage, divorce, birth or adoption of a child, loss of other coverage, or gain of other coverage. (Go to myadp.com -> Benefits -> Report Qualifying Change).
  • What if I need to make changes after Annual Enrollment ends?
    Go to myadp.com -> Benefits -> Report Qualifying Change. Please note proof of the qualifying change will be required. You will be prompted to upload the documentation.
  • I enrolled in benefits that require a payroll deduction. When does this begin?
    Deductions begin with the pay period that includes 4/1/2021. Bi-weekly pay group = 4/23/2021 Weekly pay group= 4/9/2021
  • I did not choose to enroll in this coverage, why is it listed?"
    This is a company paid benefit and eligible employees are automatically enrolled.
  • Why can I not change the coverage amount of the Basic Life Insurance?
    This is a company paid benefit and eligible employees are automatically enrolled and the amount is based on your union contract or non-union policy. You can add additional coverage by enrolling in the Supplemental Coverage.
  • I do not see a beneficiary listed, but I filled out a form year ago. Why is it not showing up on MYADP.COM?"
    This means you have not updated or reviewed your beneficiary in the last 4 years, please add a beneficiary.
  • I do not want to list a beneficiary, what do I do?"
    A beneficiary is required to complete your enrollment. You can list a charity (example: local Church), a trust, an estate, or any person you choose (example: neighbor, friend, pastor).
  • I want my life insurance split between family members. How do I do that?
    If all the family members are already listed as dependents, select Update Allocations and each dependent will pop up. Under the primary beneficiary column, enter the percentage for each person to receive. Please note it must equal 100%. For example, if you wish to have fifty percent of the life insurance go to your spouse and the remaining fifty percent divided among your 2 children, it will look like this: SPOUSE = 50%, CHILD A = 25% AND CHILD B = 25% TOTAL ALLOCATION= 100%
  • I am not currently enrolled. Can I enroll now?
    Yes, however you will be required to complete an Evidence of Insurability form (EOI) that will be emailed to you from benfits@caravanfm.com. PLEASE NOTE: Your coverage will NOT be in effect until the form is completed, turned into SunLife, and approved.
  • I am already enrolled for less than $150,000 coverage and I wish to increase it. Can I do that?"
    Answer 1--if you are already enrolled and approved for coverage, you can increase the employee supplemental life coverage by $10,000 (up to $150,000 as long as it's less than 5x your income) at Annual Enrollment, and you do not have to complete an Evidence of Insurability Form (EOI). Answer 2--if you increase coverage by more than $10,000 (up to $150,000 as long as it's less than 5x your income), you will be required to complete an Evidence of Insurability form (EOI), that will be emailed to you form benfits@caravanfm.com. Your coverage increase will NOT be in effect until the form is completed, turned into SunLife, and approved. PLEASE NOTE THE AMOUNT YOU WERE PREVIOUSLY APPROVED FOR WILL REMAIN IN PLACE.
  • I already entered a beneficiary for BASIC LIFE & ADD. Why do I have to enter it again?
    Your Supplemental coverage is a separate policy and requires a separate beneficiary designation. Your beneficiary for the Supplemental coverage does not have to be the same as the BASIC LIFE & ADD.
  • What is the difference between Primary and Secondary Beneficiary?
    The primary beneficiary is first in line to get the life insurance, if that person is deceased at the time of your death then (and only then) does the secondary beneficiary receive the life insurance funds.
  • Why will the enrollment not allow me to add Spousal Supplemental Coverage?
    Your spouse is not listed as a dependent OR You have not enrolled in Employee Supplemental Life coverage. Employee Supplemental Life must be equal to or more than the Spousal coverage amount.
  • I am not currently enrolled in Employee Supplemental Life. Can I enroll now and add Spouse Supplemental Coverage?
    Yes. You will be required to complete an Evidence of Insurability form (EOI) for the Employee coverage. The form will be emailed to you. PLEASE NOTE: Both Employee & Spouse coverage will NOT be in effect until the form is completed, turned into SunLife, and approved.
  • I am currently enrolled in Employee Supplemental Life. Can I enroll now and add Spouse Supplemental Coverage?
    Yes, you can enroll for an amount equal to or less than the Employee Supplemental Life Coverage amount up to $25,000. However, your spouse will be required to complete an Evidence of Insurability form (EOI); the form will be emailed to you. PLEASE NOTE: Spouse coverage will NOT be in effect until the form is completed, turned into SunLife, and approved.
  • My spouse is already enrolled for less than $25,000 coverage and I wish to increase it. Can I do that?"
    Answer 1---Yes, if you are already enrolled and approved for coverage, you can increase the employee supplemental life coverage by $5,000 (up to $25,000) at Annual Enrollment and you do not have to complete an Evidence of Insurability Form (EOI). PLEASE NOTE: Coverage increase will not take effect until 4/1/2021. OR Answer 2---Yes, however if you increase coverage by more than $5,000 (up to $25,000) you will be however you will be required to complete an Evidence of Insurability form (EOI) that will be emailed to you form benfits@caravanfm.com. Your coverage increase will NOT be in effect until the form is completed, turned into SunLife, and approved. PLEASE NOTE THE AMOUNT YOU WERE PREVIOUSLY APPROVED FOR WILL REMAIN IN PLACE.
  • How do I update the beneficiary for the Spouse Supplemental Coverage?
    The employee is automatically the beneficiary for Spouse Supplemental Coverage; this cannot be changed.
  • Why won't the enrollment allow me to add Child Supplemental Coverage?
    Your child is not listed as a dependent OR You have not enrolled in Employee Supplemental Life coverage. Employee Supplemental Life must be equal to or more than the $10,000.
  • I want to increase the Child Supplemental Coverage to more than $10,000. Can I do that?"
    No, the Child Supplemental Coverage is set at $10,000.
  • I have 3 children, but there is no place to add each child to the coverage. How can I do that?"
    The Child Supplemental Coverage will cover all dependent children under 19 that you have listed as dependents. The rate does not increase/decrease.
  • I am already enrolled in Employee Supplemental Coverage for $10,000 or more, and I wish to add Child Supplemental Coverage now.  How can I do that?"
    Yes, coverage will show pending until after Annual Enrollment is complete.
  • I am not currently enrolled in Employee Supplemental Life. Can I enroll now and add Child Supplemental Coverage?
    Yes, however you will be required to complete an Evidence of Insurability form (EOI) for the Employee coverage. The form will be emailed to you. PLEASE NOTE: Both Employee & Child coverage will NOT be in effect until the form is completed, turned into SunLife, and approved.
  • How do I update the beneficiary for the Child Supplemental Coverage?
    The employee is automatically the beneficiary for Child Supplemental Coverage; this cannot be changed.
  • How do I upload documents for Qualifying Event Confirmation?
